Our Transition Process

Transitioning into and out of any placement can be stressful for young people. Therefore, we aim to ensure that this process is personalised and supportive, considering any factors that may contribute to a positive placement.

Incoming

We collaborate with referring agencies to establish a process that fosters confidence and builds trusting relationships with staff, ensuring that the young person enters the placement with the best possible mindset. Our referral process revolves around maintaining excellent communication throughout and beyond the transition. We engage the child’s parents at every stage and provide all parties involved with a clear transition plan that helps the young person feel as secure as possible.

Smooth and Supportive Transitions

We ensure that every young person, their family, and referring agencies feel informed, supported, and confident as they begin their placement.

Planning for the Next Step

We help create a positive, well-informed move that sets the young person up for continued success in their next environment.

Outgoing

When the young person is ready, we work with all parties to make this complex process as smooth and successful as possible. We support visits to new settings to familiarise the young person with the buildings and staff, provide guidance to the new setting, and ensure that the new setting has as much background information as possible. We work with the new setting to build and implement a suitable transition plan, and we work with the young person before their move to ensure they are in the best possible place to make a positive transition. A full academic report is provided to the next setting, which includes progress towards EHCP targets, daily logs, and academic levels and targets reached.
